In this case, the metal track (green) that connects the handle assembly to the hook at the top of the door had disengaged.

The hook (yellow) is normally actuated by the track working a cam assembly within the grey metal casing (blue).

It is accessed by undoing the two Torx (TM) screws marked with red arrows.
